Hello Rafal,

I have an official (embossed with seal) copy of WMV's "Certificate of Death" 
(ordered from http://www.nyc.gov/html/doh/html/vr/vrdappl.shtml).   The 
entries on the form are all hand written.  The first letter 'F', the middle 
'q', and the last letter 'z' in script of his father's middle name are 
questionable. There is no city listed under place of birth.
