I do not understand what "No" is in your hypothesis.
Would it be an abbreviation of the first name?

Petr Kazil has indeed confirmed the position of the accents.

On this, I completely agree with you. And I agree with Petr Kazil too,
when he says:

Alas, the libraries I live close to do not have it.

Tichy is presently the 52nd most common surname in the Czech Republic.
