Jeff, I'm certainly an advocate for the "England" origin, but I'm just
curious why you think it originated from England.  Most evidence others are
amassing does not negate the "England" hypothesis, but "England" is
certainly in the minority right now.  I have my reasons for believing this,
will you share yours?
