I've just realised that I got the order wrong in my revised diagram (so 
will repost the corrected version in a minute, sorry) - but note that I'm 
totally convinced that the bifolio with the quire marking should be placed 
one in from the centre of the quire.

This would mean that the quire numbering would have to have been added 
*after* production, once the order had got scrambled at some stage (perhaps 
the binding came undone).

If it's John Dee's quire marking (as I suspect) and the production date is 
circa 1460 (as I suspect), then I have no problem with the two being separate.
