All I have seen is the published diploma. I suppose it is a transcript from
a copy-book ("Gedenkbuch") or similar because of the header and the lack of
signatures. In the "Hofzahlamtsbuch" his name is given as "Jacob Horzizkhi"
in 1609 and "Jacob Horschizki" in 1612 with no mention of the attribute. This
does not say too much and even in the absence of the original diploma I would
assume he has received the attribute 'de Tepenec'.
To decide Your question we have to go into detail about the reliability of
the source of the published diploma or get a signature from his time in
Melnik.
