...more likely 3 bifolios?
Oops... of course three bifolios. That would be f59r/v with 64v/r,
f60r/v with 63v/r, and 61r/v with 62v/r. That would finish off a quire
of 5 bifolios with the missing 61v-62r in the middle of the quire.
I seriously doubt the foliator calculated how many numbers to skip
at various points while numbering the pages in honour of the author,
especially
because the pages are certainly not in the original order. The missing pages
were
there at one time, when they were cut out or when they fell out is pure
speculation
at this point. The foliator simply flipped through the top right of the
codex
sitting in front of him happily numbering away without any time spent
unfolding
the folios folded in on themselves or checking to make sure the pages were
in the
right place or the quires were aligned properly, etc...

I don't know what you think, but I think there appears to be a little
bleed-across from the missing f59r onto the right hand side of f58v. What
little evidence we have surely points to the foliator having added the
foliation honestly, in line with what he/she was looking at - and therefore
to pages having been removed/lost subsequently. Why are people so paranoid
about something so straightforward?
